Getting a DVA Psychiatric Report

(for Mental Health conditions)

with Dr. Al Griskaitis

Step 3

Document your

Adversities & Trauma

There's two documents you'll need to prepare:

 "Trauma Headlines"

Just the gist of bad stuff (instructions).

"Summary of Postings"

Just a list of postings (instructions).

Step 1

Contact me!

Email me let me know what you need and your phone number

.

Step 2

Online Forms

I need some info,

use these online assessment forms:

FORM 1   (general info) &

FORM 2   (checklists).

(about 20 minutes each)

Ideally a Family/friend and psychologist/GP to also answer a few questions (5 minutes)

FORM 3   (collateral info)

Step 4

Assessment

The hard work has been done!

The last bit is the assessment, it's informal.

I try to have that booked in within a week of you supplying my the info above.

You'll need a GP referral.

We'll look at big picture stuff and treatments that might help.

I usually have both the

  • DVA report and
  • Detailed treatment plan

for your GP ready the next day.

If you'd like to have a DVA report prepared by Dr. Al, you've come to the right page.

I do DVA reports all the time!

My reports are bomb-proof and I can get them done really quick!

There can be hold ups in getting the DVA REPORT REQUEST, so get on DVA's radar:

compensation.claims@dva.gov.au

First Up

Before I can do a report:

  1. We'll need to have a email check in or phone call to sus out what you need. Email me: al@thepsychcollective.com
  2. We need a request for a report from DVA. If you don't have one, there's some paper work that might need to be done.

Watch the Video explainer above

Brief instructions:

Complete the online FORMS:

  1.  Complete FORM 1 (about 20 minutes):

> Link to FORM 1: Background information

  2. Complete FORM 2 (About 20 minutes):

> Link to FORM 2:  Symptoms and severity

  3. Send the link for FORM 3 (https://forms.gle/sZ8UjqiGoCGmiMxS8) to people who can corroborate your impairment (eg GP, colleague, family, psychologist, etc. not essential, but helpful.)

Prepare TWO documents:

  1. SERVICE-RELATED TRAUMA (use this template: word doc) CRITICAL
  2. SUMMARY OF POSTINGS (use this template: word doc or pdf) This is helpful, but less critical

Email them to me!!

How quickly that's done is on you. As soon as you've done these steps we'll book in a phone appointment, usually within a week.

I've turned around reports in 24 hours for people who get everything sorted fast. I usually do phone assessments on Thursdays, report done by Friday.

The sooner you get your bits done, the sooner we can have the report prepared (the biggest delay is getting the request from DVA).

Personalised, Independent Support for Your DVA Claim

When you work with me, you’re not dealing with a shonky third-party provider nor someone affiliated with DVA. You’re dealing directly with an experienced psychiatrist who focuses on PTSD and understands the challenges you’ve faced in service and the consequences of those challenges.

My goal is simple: to ensure your sacrifices are appropriately recognised and that you receive the full entitlements you deserve. I bring my expertise and a deep commitment to your case, crafting detailed, evidence-based reports tailored to meet DVA requirements and support your claim.

You’ve given your best in service—now it’s time to make sure you’re supported in return.

Best wishes,

- Dr Al Griskaits

If someone else is doing the report for you, check out this blog about getting your conditions related to service, it explains heaps about what needs to go into a solid report.
