First Up
Before I can do a report:
- We'll need to have a email check in or phone call to sus out what you need. Email me: al@thepsychcollective.com
- We need a request for a report from DVA. If you don't have one, there's some paper work that might need to be done.

Brief instructions:
Complete the online FORMS:
1. Complete FORM 1 (about 20 minutes):
> Link to FORM 1: Background information
2. Complete FORM 2 (About 20 minutes):
> Link to FORM 2: Symptoms and severity
3. Send the link for FORM 3 (https://forms.gle/sZ8UjqiGoCGmiMxS8) to people who can corroborate your impairment (eg GP, colleague, family, psychologist, etc. not essential, but helpful.)
Prepare TWO documents:
- SERVICE-RELATED TRAUMA (use this template: word doc) CRITICAL
- SUMMARY OF POSTINGS (use this template: word doc or pdf) This is helpful, but less critical

How quickly that's done is on you. As soon as you've done these steps we'll book in a phone appointment, usually within a week.
I've turned around reports in 24 hours for people who get everything sorted fast. I usually do phone assessments on Thursdays, report done by Friday.
The sooner you get your bits done, the sooner we can have the report prepared (the biggest delay is getting the request from DVA).
