We know that the hardest part of starting psychological treatment is often the need to tell your story over and over again. Every time you see a new clinician, they ask the same questions and you begin to feel like a broken record.
At The PSYCH Collective, we have created an online assessment tool that helps to gather some background information before you undertake an initial appointment with a care provider (psychologist or psychiatrist).
This survey is FREE to use and will help your treatment provider to get a sense of your symptoms and your background. This means that your clinicians only needs to ask clarifying questions that are relevant to the information you have provided in this survey, rather than a battery of questions.
It takes 15-30 minutes and people usually find the process helpful.
The report goes to your email and will be emailed to your treatment provider if they are registered with us (and if you nominate them).
We have created a sample report for you to download so you can see what sort of information is collected and decide if this will be useful for you.
